Would a company that runs a Bitcoin ETF have to buy Bitcoin or would they sell unbacked IOUs?

When Blackrock and other companies finally get the green light to run a Bitcoin ETF, how would that work?

When investors put money into the ETF, do they buy Bitcoin for it? Or do they do nothing and just tell you "If you want to sell your shares again, we'll by them for a price that matches the ROI you would have had if you invested in BTC"?
